Form A is the registration form for religious formation grades Kindergarten through 10th grade for 2026-2027. Feel free to print out and turn in at Parish Office. Registration dates have not yet been set but forms are welcome at any time. Other requirements are required for registration especially for those children registering for Sacraments.
